What Is Move Up?
MOVE UP is a new program of Heron’s Gate that helps struggling to launch young adults start building a foundation for independent young adulthood. It focuses on providing the support, training and practice that will help your young adult develop basic skills in three key areas:
- Life skills
- Social skills
- Emotional self-management
Move Up does not specifically treat the mental health needs of your young adult, such as depression or ADHD. If needed, those services are available through a more intensive, higher level program within Heron’s Gate.
Applying To Move Up
How Does My Young Adult Get Into Move Up?
If you are interested in applying to the Move Up program, please download and complete this screening form: Screener For Move Up Program. This information lets us determine if your young adult is more appropriate for the skills-based approach of Move Up, or might benefit from more intensive Heron's Gate services.
Once completed, scan and e-mail the form to Rose Cohen at email@example.com. She is your first point of contact for the program. You can also call her for additional information at 410-740-3240.
The form will be reviewed by the Move Up application team. You will then be contacted by a clinician about your application status and to discuss the most appropriate treatment option.
